Learn about how to request a birth certificate or a copy of medical records from Medical City Plano hospital.

Certified Birth Certificates

When a child is born at Medical City Plano, we submit the birth certificate information to the Bureau of Vital Statistics. Certified birth certificates can only be obtained through the bureau, which is part of the Texas Department of State Health Services. The hospital does not have a certified copy on file.

Obtaining a Certificate

For parents or patients over the age of 18: To obtain a birth certificate, submit an application to the Bureau of Vital Statistics. You can do this one of three ways:

Copies of Medical Records

To obtain a copy of your medical records, complete the Release of Information (ROI) Form (To be included on Medical Records page), and provide a copy of your driver's license. Submit the information one of two ways:
